HOW IT WORKS
How CDHCI works

AHS assessment
Call 811 or speak with your physician to request an Alberta Health Services needs assessment.

Authorization
An AHS case manager determines eligibility and your approved monthly care hours.

Choose KindHavn
Select KindHavn as your approved CDHCI provider. We handle the Alberta Blue Cross paperwork.

Care begins
Once your approval letter is received, your care starts on your schedule.
ELIGIBILITY
Do you qualify?
Eligibility is determined by Alberta Health Services, not by KindHavn. If you're unsure whether you qualify, call 811 to request an assessment or speak with your family physician.
Alberta resident with a valid Alberta Health Care Card
Assessed and approved for home care by Alberta Health Services
Needs can be safely met in a home environment
Able to manage or direct your own care, or have someone to do so on your behalf

Not yet assessed? We'll walk you through how to request an AHS assessment and what to expect.
